How to File a Claim

At First Western Insurance, we understand the urgency of submitting claims promptly and are committed to processing upon receipt.

During business hours, please submit a completed Acord Notice of Loss form and supporting documentation (i.e. repair estimate or invoice, police report, etc.) to the Claims Department via email or fax as soon as possible. For assistance in completing an Acord Loss Notice, go to www.simply-easier-acord-forms.com.
  • Normal business hours are 7:30 a.m. to 4:30 p.m. Monday through Thursday and 7:30 a.m. to 3:15 p.m. Friday.
Email claims to apps@fwins.com or
fax to (515) 225-3872.

Upon receipt of the Claim Notice, we will forward the loss information to the carrier. We will notify the retail agent of the assigned claim number and adjuster contact information upon receipt from the carrier.
